AccessOne® for STELARA cannot accept any information or honor any request for coordination of nursing services without an executed Business Associate Contract (BAC) with no Limitation of Services, or a Patient Authorization (PA) form on file. The Patient Authorization (PA) for STELARA can be found on the Prescription Information, Nursing Order and STELARA Support Enrollment Form. In addition, a completed Prescription Information, Nursing Order and STELARA Support Enrollment Form must be submitted for each patient for whom coordination of medication delivery and coordination of nursing services for treatment with STELARA is requested.

A Prescription Information, Nursing Order and STELARA Support Enrollment Form, with sections completed for coordination of medication delivery and nursing services, must be submitted to AccessOne® by the prescriber in order to activate AccessOne® Nurse Coordination. No other forms for request of AccessOne® Nurse Coordination will be accepted. AccessOne® Nurse Coordination cannot be requested directly by the patient.

AccessOne® Nurse Coordination is not available to patients participating in a Patient Assistance Program (PAP), because STELARA is shipped to the physician's office. Certain restrictions on program availability may apply.
