Introduction

It is common for public and commercial payers to create guidelines to address billing and reimbursement for healthcare services. These guidelines dictate which and under what circumstances products or services are covered and often which billing codes to include on the claim form for those products or services. If the guidelines are not followed, the claim may be denied. Coverage may depend on criteria such as the indication, site of service, and clinical laboratory ranges, all of which should be outlined in the guidelines. As reimbursement and billing guidelines vary by payer, it is important to contact the payer directly or consult their websites to obtain product-specific coverage and reimbursement information.
